Dr. Vinod Balachandran: Developing Personalized Cancer Vaccines for Pancreatic Cancer

Dr. Vinod P. Balachandran is a New York-based surgeon-scientist whose work is helping explore a new frontier in cancer treatment: personalized vaccines designed to train the immune system to recognize and attack a patient’s own cancer cells.

Balachandran is a hepatopancreatobiliary surgeon and physician-scientist at Memorial Sloan Kettering Cancer Center (MSK), where he serves as Director of the Olayan Center for Cancer Vaccines. He specializes in cancers involving the pancreas, liver, gallbladder and bile ducts and combines surgical care with laboratory and clinical research. He earned his medical degree from SUNY Stony Brook, completed general surgery residency at NewYork-Presbyterian Hospital and trained in complex surgical oncology at MSK.

Much of Balachandran’s research focuses on pancreatic cancer, one of the most difficult cancers to treat. His team is investigating whether individualized messenger RNA (mRNA) vaccines can stimulate a patient’s immune system to recognize specific mutations found in their tumor.

The approach is highly personalized. Rather than creating one vaccine intended for every patient, researchers analyze an individual’s tumor to identify mutations that may produce recognizable targets for the immune system. A vaccine can then be designed around those targets. MSK describes this approach as a form of precision immunotherapy.

In 2026, Balachandran and his colleagues reported encouraging long-term follow-up from an early clinical trial. Among the 16 participants who received the personalized RNA vaccine, eight generated a strong vaccine-induced immune response. Six years later, seven of those eight responders were still alive. The researchers emphasized that these are early findings from a small Phase 1 trial and require confirmation in larger studies.

The results are significant because pancreatic cancer has historically been particularly resistant to immunotherapy. The research suggests that carefully designed vaccines may be capable of generating long-lasting immune activity against pancreatic tumors.

The work is now moving into larger clinical testing. MSK is conducting a Phase 2 study evaluating an mRNA vaccine combined with immunotherapy and chemotherapy against standard chemotherapy alone in people with operable pancreatic cancer. Balachandran is the study’s investigator.

His contributions have also earned significant recognition. In 2025, Balachandran was named to the TIME100 Health list and received the Marcus Foundation Medical Research Award for work on next-generation precision RNA vaccines.

What makes Dr. Balachandran especially notable is the combination of surgery, immunology and personalized medicine in his work. His research represents an effort to move cancer treatment beyond simply attacking tumors and toward teaching the patient’s immune system to recognize and remember cancer.

Although the vaccines remain investigational and are not yet standard treatment, the continuing research offers an encouraging example of how personalized medicine and immunotherapy could reshape cancer care. Dr. Balachandran’s work at Memorial Sloan Kettering places him among the U.S. physicians exploring some of the most promising possibilities in modern oncology.
