Few neurological diseases are as challenging as amyotrophic lateral sclerosis (ALS), a progressive condition that damages nerve cells responsible for movement. At Johns Hopkins University, Dr. Nicholas Maragakis has spent much of his career working to better understand ALS and develop new approaches that could eventually lead to improved treatments.

Dr. Maragakis is a Professor of Neurology at Johns Hopkins University School of Medicine, Director of the Johns Hopkins ALS Center for Cell Therapy and Regeneration Research, and Co-Director of the Johns Hopkins ALS Clinic. His work combines patient care with laboratory research aimed at understanding how ALS develops and progresses.
A particularly important part of his research involves understanding the role of cells surrounding motor neurons. His laboratory has investigated astrocytes and other glial cells, which were once viewed primarily as supporting cells but are increasingly understood to play important roles in neurological disease.
Maragakis and his colleagues have also used induced pluripotent stem cells (iPSCs) derived from ALS patients. These cells can be transformed into different types of human cells, giving researchers a way to study disease mechanisms in laboratory models that more closely reflect human biology. His laboratory helped create stem cell lines from ALS patients as part of this research effort.
This work is particularly significant because ALS has historically been difficult to study. Researchers need to understand not only what happens to motor neurons, but also why surrounding cells may contribute to their deterioration. Maragakis’ research has examined how astrocytes may influence the spread and progression of ALS and whether these pathways could become targets for future therapies.
His research program is also closely connected to clinical trials. As Medical Director of the Johns Hopkins ALS Clinical Trials Unit, Maragakis helps connect laboratory discoveries with the development and evaluation of potential therapies. The Johns Hopkins ALS Center’s broader mission is to identify the causes of ALS and discover new treatment options for patients.

In addition to his research, Maragakis has spent more than two decades caring for patients with ALS. That combination of clinical experience and laboratory investigation gives his work a particularly practical focus: discoveries made in the laboratory ultimately need to translate into meaningful improvements for people living with the disease.
